The seat of the Catholic Diocese of Urdaneta, in Barangay Poblacion. It is dedicated to Our Lady of the Immaculate Conception, who is also the city patron. Father Rafael Cano began the building on 4 March 1884, and it opened with three naves, 77.10 metres long and 23.30 metres wide. Bombing in the Second World War damaged it. The retablo is the work of the Kapampangan carver Willy Layug, who also made the ones in Dagupan, Bacolod and San Jose in Nueva Ecija.
The city fiesta runs from 1 to 8 December in her honour, with a parade, a drum and lyre competition, a coronation night for Miss Urdaneta City and a carnival that stays up for the month.
